Medical Treatment Authorization
In the event of a medical emergency where I cannot be reached, I authorize camp staff and medical personnel to administer first aid, CPR, and to arrange transport to the nearest medical facility. I authorize licensed medical professionals to provide necessary emergency treatment, including but not limited to hospitalization, surgery, and anesthesia.
