Every published form includes a report option. If someone encounters a form that looks suspicious, fraudulent, or is being used for phishing — they can report it directly to our team for immediate review.

Proactive Monitoring

We actively monitor for patterns associated with phishing, scam, and spam forms. Suspicious activity is flagged automatically and reviewed by our team to protect respondents and maintain platform integrity.

Zero Tolerance Policy

Any form found to be used for illegal purposes — phishing, fraud, identity theft, or scams — is taken down immediately. The associated account is suspended without notice and permanently banned.

How We Handle Violations

Our enforcement process is clear and swift: (1) Investigate the reported form, (2) Take down the form immediately if confirmed, (3) Suspend the user's account, (4) Cooperate with law enforcement authorities when required.

If you've encountered a form on FormBuilder that looks suspicious, is being used for phishing, fraud, or any illegal purpose — please report it immediately.

How to Report a Form

Send an email to info@formbuilder.ai with the subject line Form Report and include:

The URL of the suspicious form
A brief description of the issue
Screenshots if available
How you encountered the form (link, email, social media, etc.)

Our team reviews every report and takes action within 24 hours. Confirmed violations result in immediate form takedown and account suspension.
