This root canal therapy and endodontic treatment informed consent form gives dental practices a clear, structured way to document patient consent before endodontic care. It collects patient details, the tooth number(s) to be treated, the planned procedure, the treating dentist or endodontist, the anesthesia or sedation plan, and any known allergies to medications, anesthetics, or latex. A built-in, plain-language explanation then walks patients through how root canal therapy works, its benefits, possible risks and complications, available alternatives, and important aftercare, including the need for a prompt permanent crown or filling.

The template is designed for general dentists, endodontists, dental clinics, and group practices that want to replace paper consent packets with a consistent digital process. Patients confirm their understanding through individual acknowledgment statements covering the purpose of treatment, risks, alternatives, post-operative responsibilities, and full disclosure of their medical history. The authorization section records who is signing and includes a field for a parent, guardian, or representative signing on behalf of the patient.

Using an online consent form helps your team save chair time, reduce missing paperwork, and keep a reliable record of informed consent for every patient. Patients can review the information at their own pace before their appointment, which encourages better questions and fewer surprises on treatment day. Customize the wording, add your practice branding, adjust procedure and anesthesia options, and share the form by link, email, or an in-office tablet to make root canal consent fast, thorough, and easy to manage.

Understanding Your Root Canal Treatment
1 What Is Root Canal Therapy? Endodontic (root canal) treatment is recommended when the pulp becomes inflamed, infected, or dies. The pulp is the soft tissue inside the tooth that contains nerves and blood vessels. Common causes are deep decay, a crack, trauma, or repeated dental procedures. After the area is numbed with local anesthesia, the dentist makes a small opening in the tooth and removes the damaged pulp. The dentist then cleans, shapes, and disinfects the root canals before filling and sealing them. Treatment may require more than one visit, and radiographs (X-rays) are taken before, during, and after the procedure. Benefits: Relief of pain and infection while preserving your natural tooth. Possible Risks & Complications: Post-operative pain, tenderness, or swelling; flare-up of infection; jaw soreness; a separated (broken) instrument in a canal; root perforation; calcified or curved canals that cannot be fully treated; tooth or root fracture; damage to existing crowns, fillings, or bridges; temporary or, rarely, permanent numbness of the lip, chin, or tongue; and reactions to anesthetics or prescribed medications. Although most root canal treatments are successful, some teeth may later require retreatment, endodontic surgery (apicoectomy), or extraction. Alternatives: Extraction (with optional replacement by an implant, bridge, or partial denture), delaying treatment to monitor the tooth, or no treatment. Delaying or declining treatment may lead to worsening pain, spreading infection, bone loss, and loss of the tooth. After Treatment: A temporary filling is placed. The tooth will need a permanent restoration (often a crown) as soon as possible to protect it from fracture and reinfection. If oral or IV sedation is used, a responsible adult must drive you home, and you should not drive or operate machinery for at least 24 hours. richtext
