1 Media Release Agreement: By signing below, I grant our church, its staff, volunteers, and ministries permission to photograph, video record, audio record, and livestream the participant during worship services and ministry events. I also grant permission to use these materials according to the preferences selected on this form. I understand that all media is used solely for worship, ministry, communication, and outreach purposes. I understand that no compensation will be provided and that all photographs and recordings remain the property of the church. I may update or revoke this consent at any time by written notice to the church office.
