CONDITIONAL WAIVER STATEMENT: Upon receipt and clearance of the progress payment identified above in the amount stated, the undersigned Subcontractor hereby conditionally waives and releases any and all lien, stop-payment notice, and payment bond rights the undersigned has against the property, the general contractor, the owner, and the construction lender for labor, services, equipment, and materials furnished through the billing period end date stated above, except for any disputed amounts or exceptions noted. This waiver and release is conditioned upon actual receipt of payment. If payment is not received, or if the instrument of payment is dishonored, this waiver and release shall be null and void.
